At present, treatment of vtec infection is primarily supportive. A small prospective placebo controlled study to evaluate the effect of trimethoprim-sulfamethoxazole in children with proven E coli 0157:H7 infection did not show a statistically significant effect of treatment on progression of symptoms, fecal pathogen excretion or incidence of hus. However, in vitro evidence indicates that verotoxin production or release is increased after exposure to antibiotics, suggesting that antimicrobial therapy in vivo may not be beneficial.
